چكيده طي سال هاي گذشته با بالا رفتن هزينه بهداشت و درمان در اكثر كشور ها و اهميت كاركرد صحيح اين مراكز به علت رابطه مستقيمي كه با جان انسان ها دارد، توجه زيادي به بخش هاي مختلف آن شده است. يكي از بهترين ابزار ها و متودولوژي هاي بهبود، اصول و تفكر ناب است كه همواره با حذف اتلاف و ايجاد حركت پيوسته سعي در ارزش آفريني در سيستم دارد. لذا با اندازه گيري ميزان عملكرد بيمارستان بر پايه اصول ناب و شناسايي عواملي كه مانع اجراي اين رويكرد در مركز هستند و امتياز دهي به آنها، مي توان مراكز بهداشتي و درماني را به سمت اجرا اصول و تفكر ناب هدايت نمود. در اين پايان نامه سعي بر آن شده است كه مدل ارزيابي عملكرد سريع گودسان براي مراكز بهداشتي و درماني توسعه داده شود. اين مدل بر خلاف ساير مدل هاي ارزيابي عملكرد نيازمند داده هاي فراوان اوليه و صرف زمان زياد نمي باشد و در كمترين زمان ممكن به مديران كمك مي نمايد تا بازخورد خوبي نسبت به عملكرد مركز خود بدست آورند. در دپارتمان اورژانس به عنوان يكي از قسمت هايي از بيمارستان به علت محدوديت هاي زماني براي درمان و كمبود تجهيزات و امكانات در زمان هاي ازدحام اتلاف نسبتا زيادي اتفاق مي افتد.در عين حال اين مراكز به دليل حساسيت بسيار بالاي كار و فعاليت هاي بحراني خود نيازمند بهترين عملكرد و بهره برداري از منابع موجود مي باشند بنابراين در پژوهش پيش رو سعي بر آن شده است تا مدل ارزيابي عملكرد سريع گودسان كه بر اساس اصول ناب پايه گذاري شده است جهت اندازه گيري عملكرد دپارتمان اورژانس توسعه يابد. بدين منظور بعد از توسعه مدل بر اساس اصول و ارزش هاي تفكر ناب در مراكز بهداشتي و درماني و طراحي فرم هاي ارزيابنده جهت بررسي صحت مدل دپارتمان اورژانس يكي از بيمارستان هاي بزرگ شهر تهران مورد ارزيابي عملكرد قرار گرفت. بعد از بازديد امتياز دهي و تحليل نتايج پيشنهاداتي مبني بر بازنگري سيستم زنجيره تامين و تعميرات نگهداري تجهيزات مركز اورژانس، داده شد. واژههاي كليدي: ارزيابي عملكرد، دپارتمان اورژانس، ارزيابي عملكرد سريع، اصول ناب

Abstract Over the past years, with the rising cost of health care in most countries and the importance of proper functioning of these centers due to their close relation with human life, much attention has been paid to different parts of the field. One of the best tools and methodologies for improvement is lean principles and thinking which always aim to create values in the system by eliminating losses and creating continuous motion. Therefore, with the measurement of hospital performance based on lean principles and identification of the factors which function as obstacles to the execution of this approach in the aforementioned centers and also by rating them, health centers can be led toward application of the principles of lean thinking. This study has aimed to develop Goodson’s rapid performance assessment for health centers. Unlike other performance eva​luation models, this model does not require abundant primary data and long periods of time and in the shortest possible time it helps managers to receive optimum feedback towards their performance. In the emergency department as one of the hospital units, due to time constraints and lack of treatment facilities in times of congestion, a relatively large loss occurs. At the same time because of their high sensitivity and critical activities, these centers require the most optimum performance and operation of existing resources. Therefore, this study has aimed to develop Goodson’s rapid performance eva​luation model which is based on lean principles for the measurement of the performance of emergency departments. Therefore, after development of the model based on lean thinking principles and values in health centers and designing eva​luation forms in order to eva​luate the validity of the model, the performance of the emergency department of a large hospital in Tehran was investigated. After observation, scoring and analysis, the proposed results for the revision of supply chain systems and repair equipment Emergency Center were presented.
