Regarding approved upstream documents, Inpatient treatment service purchasing from non-governmental hospitals, in order to decrease government’s trunk and also limiting public sector’s responsibility to stewardship, could be considered as a model with more cost-efficiency for government and increasing poor people’s contentment in receiving high quality services along with reducing financial burden in areas lacking public hospital. In this study, a comparison considering quality, contentment and economic efficiency was done between providing inpatient treatment services directly by a governmental hospital and purchasing services from a private hospital which aimed to assess service purchasing model efficiency.
Public hospital of“Amir-Al-Momenin” in Bouyin-Zahra (Qazvin province-I.R.Iran) and Private hospital of “Vali-e-Asr” in Abyek city (the same province) as the provider which its services were perchased by the government, were compared for quality, contentment of patients and personnel and government expenditures. We assessed quality and contentment using questionnairs and economic analysis was done by measuring all allocated resources and registered expenditures in two hospitals. Results were statistically analysed considering P-value less than 0.05.
Although general contentment did not show any significant difference between two models; paurchasing services from private sector in two consecutive years of 2015 and 2016 showed 18 to 29 percents of financial saving for the goverment.
Because of economic efficiency and generally acceptable contentment, the model of purchasing inpatient treatment services from non-governmental sector could be a preferable option compared to direct service providing by government. With regard to some boundries and resistance against implementation of this model, necessity of training all authorities in national and local levels in addition to developing its culture, is felt.
