The supply chain is an important network in the infrastructure of any company's business so that it can deliver the product or service to the customers in the shortest time and in the best possible way. This network includes suppliers, manufacturers and distributors and retailers and finally customers. Delay or disruption in any node of the supply chain network causes a lot of costs for the business system of any company. The packaging industry has a direct relationship with the protection, transportation and marketing of any product and the branding of companies in this sense, disruption in the supply chain of the packaging industry will have a direct effect on other companies. Therefore, by knowing the important risks of each part of the supply chain, it is possible to provide the managers, experts and specialists of the supply chain network with packaging essentials, a complete view of the conditions in the entire supply chain. In other terms, the economic, social, and environmental issues of this industry are very important, and the maintenance of sustainability risks in the supply chain of the packaging essentials industry will be investigated. In the first stage, the risks that can disrupt the supply chain network of the packaging industry's essentials are identified through the review of literature and theoretical foundations in this thesis, there are 12 main criteria and 172 sub-criteria and in the second stage, in order to prioritize each risk and identify potential risks, a questionnaire is distributed among experts to give qualitative points to these risks and to identify important risks. After collecting the questionnaires, the inconsistency rate index of each specific questionnaire is calculated and after the final approval of each questionnaire with the improved fuzzy hierarchical process (AHPF) method, the weight of each risk is quantitatively calculated and potential risks are extracted. Due to the existing uncertainty of preference judgments and prioritization of alternatives, the fuzzy numbers approach has been used in the questionnaires, and in this research, due to the fact that the information is collected for descriptive conditions, triangular fuzzy numbers are used. In the third stage, after the identification of potential risks, a questionnaire will be presented to the supply chain experts in order to provide appropriate strategies for potential risks. In the fourth step, to identify the sensitivity of risk parameters, experts are divided into three categories of decision-makers, which will include risk-taking decision-makers, neutral decision-makers and risk-averse decision-makers considering these three decision makers, a sensitivity analysis will be performed on parts of the main risk criteria and the degree of sensitivity of the risks that are important in the packaging essentials industry will be examined.
