Hospitalization and treatment costs increased also limitation of hospital beds and population number have changed patient care system from hospitals to patient's home. Meanwhile, increasing of cancer disease cases makes a need of more patient care in hospital. One of the options is hospitalization and care of patient in their home which will reduced treatment costs and improved their quality of life.
This thesis aims to provide a model for planning a medical team visit for cancer patients in their own home, taking into account the criteria such as continuity of treatment, reducing the time of routes, and improving the quality of service delivery which will be the outcomes as well. The planning is based on the nature of the vehicle routing problem, which was at first modeled on the mixed-integer problems, and then, due to the high-solution time, the innovative algorithm of an adaptation large neighbor searched.
The length of the main planning horizon is one week, but due to the very high changes in the patients' situation and the uncertainty in the demand for modeling, the planning horizon has considered for two days. Although the planning was for two days as the present and the next day, but research team tried to consider the criteria for one week in the plan.
The home care system planning, taking into account all the changes in the demand and the condition of the patients, and with the use of online traffic information, has created software for the management and planning of the " Ala Cancer Control and Prevention Center". The results of this decision support system represent to increase in the planning ability and ease of planning up to 70% and also a 50 to 30% reduction in the return time of the medical team compare to the manual planning mode, which is followed by a reduction to significant costs of transport and especially manpower has been taken.
