Today, world statistics and in particular, Iran show that cancer as the second leading cause of death has become one of the main challenges in society. Breast cancer is also the first cancer among women and one out of every 7 Iranian women is involved in this issue and in terms of psychology, sociology and economics, it imposes huge costs on the individual, society and country. If with very simple initiatives and measures, this disease can be prevented up to 70%. The present study, in line with prevention policy, has designed a health innovation system in the field of cancer based on institutional mapping and GIS. The present research is descriptive-applied in terms of purpose and descriptive and survey in terms of data collection method. The research community is Iran. In the first step, using the meta-combination method, various factors affecting cancer were identified from different studies. Then, in the form of semi-structured interviews, the Q method was used to categorize mindsets and the grounded theory method was used to enumerate more factors and present a theoretical model. This step forms the structure of the multi-ground theory method. Then, field data were collected using quantitative information and satellite images for different parts of the country. Using the obtained data for the years 1384, 1390 and 1396, and Markov LCM-CA method in GIS environment, the situation of the provinces of the country until 1410 was predicted. It was determined that until the mentioned period, the cities of Gorgan and Arak as critical cities of the country will be affected by this disease. Three methods (two quantitative methods and one qualitative method) were used to validate the forecast and also machine learning-based methods were used to extract the key factors influencing the design of comprehensive national scenarios with a crisis prevention approach. Using the obtained indicators, sensitivity analysis was performed on critical provinces to ensure effective prevention policies. The functions of the problem-based innovation system were prioritized using decision-making methods so that their importance in the proposed policies would have a special weight and become clearer. One of the main policy recommendations is the establishment of centers for early detection and prevention of breast cancer, which were located in two critical cities. Finally, policy recommendations for breast cancer prevention were presented in the form of problem-based innovation system functions. Institutional mapping was used based on policy recommendations, and in order to implement and operationalize it. The main institutions were identified and executive instructions were provided for the formation of working groups and committees. Finally, in order to validate the presented policies and institutional mapping, the opinions of 58 scientific and experimental experts related to different institutions were used using the previous evaluation method and peer evaluation.
