در هر جامعه¬اي مراكز بهداشت و درمان به طور مستقيم در سلامت افراد آن جامعه دخيل هستند و دسترسي آسان به خدمات بهداشتي و درماني با كيفيت مطلوب از اهميت ويژه¬اي برخوردار است. مراقبت در منزل عبارت است از ارائه خدمات بهداشتي و درماني در محل زندگي بيماران. در اين سيستم، بيمار تحت نظر پزشك و پرستار در محل زندگي خود از خدمات بهداشتي و درماني بهره¬مند مي¬گردد. در اين پژوهش، مسيريابي و زمانبندي كادر درمان براي درمان بيماران درب منازل آن¬ها با در نظر گرفتن انواع محدوديت¬ها و شرايط دنياي واقعي مورد مطالعه قرار گرفته‌ است. تمركز اصلي بر روي ارائه سرويس¬هاي مطلوب به بيماران در زمان مناسب مي¬باشد. از دستاوردهاي اين پژوهش مي¬توان به ارائه مدل¬هاي جديد با در نظر گرفتن مهارت¬هاي درمانگران و نيازهاي بيماران، احترام به ترجيحات بيماران و كادر درمان، توالي ويزيت و درمان برخي از بيماران توسط درمانگران مختلف اشاره كرد. علاوه براين، اغلب معاينات و ويزيت¬ها در منازل بيماران حساس به زمان هستند كه در پژوهش حاضر اين موضوع لحاظ گرديده است. در اين پژوهش، ضمن تشريح اهميت مسئله، با ارائه مدل¬هاي مختلف و الگوريتم¬هاي فراابتكاري به حل مسائل متعدد در حوزه مراقبت در منزل پرداخته شده است. همچنين اين مسئله، در يكي از مراكز مراقبت درمنزل شهر تهران به عنوان كاربرد عملي مورد بررسي قرار گرفته و نتايج حاصل از بررسي نمونه موردي، تأييدكننده نتايج پياده¬سازي مدل براي داده¬هاي استاندارد مورد استفاده است. اين پژوهش، يك راهنماي علمي براي تصميم¬گيرندگان مراكز مراقبت در منزل است تا تصميمات بهينه¬اي براي مسيريابي و زمانبندي كادر درمان خود انجام داده و بتوانند ترجيحات و رضايتمندي درمانگران و بيماران را نيز در نظر بگيرند. همانگونه كه نتايج نشان مي¬دهد، بكارگيري مدل¬ها و رويكردهاي ارائه شده در اين پژوهش مي-تواند منجر به كاهش قابل توجهي در هزينه¬هاي مراكز مراقبت در منزل گردد.

Providing a new approach to health systems and home health care

In any society, health care centers are directly involved in the health of individuals, and provision of easy access to health services is of great importance. Home health care generally refers to a system of providing medical care to patients at their homes. In this system, patients benefit from medical and support services performed by skilled health workers. This system is desirable for patients as well as their families because it makes the patient to return to his normal life more quickly. In this study, it was attempted to investigate the routing and scheduling of health workers for the treatment of patients at their homes considering different constraints and real-world conditions. To plan this system, patients requiring services are assigned to health workers with suitable skills and optimal routes are mapped for the health workers. The innovations of this research are the provision of new models considering the skills of health workers and the needs of patients, respect for the preferences of patients and health workers, the sequence of visits and the treatment of some patients by different health workers, etc. In addition, most of the visits at patients' homes are time sensitive, which is considered in the present study. In this thesis, in addition to the importance of the problem, various problems in the field of home health care are described. In the following, different models and meta-heuristic algorithms are presented to solve the described problems. Moreover, the problem has been studied as a practical application in one of the home health care centers in Tehran and the results of the case study confirm the accuracy and applicability of results for standard data. This research is a scientific guide for the decision-makers of home health care centers to make optimal decisions on routing and scheduling of their health workers and to be able to consider the preferences of health workers and patients. It is worth noting that, the application of the models and approaches presented in this study can lead to a significant reduction in the costs of home care centers.
